Instructions for filling in the application form:
1. Please write the contact details of the applicant / legal representative on the 1st page of the application.
2. Read the full text of the application form carefully.
3. Do not fill the other information in the application form (we will do it together afterwards).
4. Sign the application form on page 5 and fill the entire Affidavit, please.

5. Please send us the application form via e-mail to asistent@speak.sk, or call us at 0910315695 for more information. Afterwards we’ll need the original application form to be delivered to the premises of our SPEAK Language School in Banská Bystrica.
